The 2010 annual member dues for the Hawaii Immunization Coalition (HIC) is $10 per member. Members will receive immunization information and alerts via email and discounts to HIC workshops/seminars. HIC accepts credit cards, cash, checks, and purchase orders for member dues. For credit card payments, click on the Donate button on this page to pay your $10 membership fee. Complete the online membership form at www.immunizehawaii.com/joinhic.php or go to Join HIC on this page.
Checks may also be mailed to HIC at P.O. Box 25665, Honolulu, HI 96825. You will receive an email receipt.
